
A Guide to Surgical Menopause and Recovery

Surgical menopause can feel abrupt in a way that natural menopause rarely does. One week you may have functioning ovaries producing hormones; the next, following surgery, symptoms such as hot flushes, anxiety, poor sleep or vaginal dryness can appear quickly and intensely. This guide to surgical menopause explains what happens, what support may help, and why a personalised plan matters.
What is surgical menopause?
Surgical menopause happens when both ovaries are removed, usually during an operation called a bilateral oophorectomy. The ovaries are a major source of oestrogen, progesterone and testosterone before menopause. Their removal causes hormone levels to fall rapidly, bringing on menopause immediately if you had not already reached it.
This is different from having a hysterectomy alone. A hysterectomy removes the womb, so periods stop, but if the ovaries remain in place they may continue to produce hormones. Some women do experience menopause earlier after hysterectomy, even when their ovaries are conserved, but it is not automatically surgical menopause.
Ovaries may be removed for several reasons, including ovarian masses, severe endometriosis, recurrent cysts, cancer treatment or a significantly increased inherited risk of ovarian cancer. The reason for surgery matters when planning treatment afterwards, particularly where there is a current or previous hormone-sensitive cancer or complex endometriosis.
Why symptoms can be more intense
In natural menopause, ovarian hormone production usually declines over a number of years. Surgical menopause does not offer that gradual transition. The sudden reduction in oestrogen can affect temperature regulation, sleep, mood, concentration, joints, skin, bladder and vaginal tissues. Reduced testosterone can also contribute to low sexual desire, reduced arousal, fatigue and a loss of motivation for some women.
Symptoms vary considerably. Some women notice very little at first, while others develop severe flushes, night sweats, panic symptoms or insomnia within days. The physical recovery from surgery can also overlap with hormonal symptoms, which can make it difficult to separate pain, exhaustion and emotional changes in the first few weeks.
The emotional impact deserves proper attention too. Surgery may have followed years of debilitating symptoms, a cancer-risk decision, an unexpected diagnosis or a difficult fertility journey. Feeling relieved and grieving at the same time is common. If the surgery takes place before you had expected menopause, the sense of change can be especially profound.
The longer-term health considerations
Oestrogen has effects beyond periods and flushes. When menopause happens early, especially before age 45, lower oestrogen exposure over many years may increase the risk of bone loss and osteoporosis. It can also affect cardiovascular health, metabolic health and urogenital symptoms such as recurrent urinary infections, vaginal discomfort and urinary urgency.
This does not mean that poor health is inevitable after ovary removal. It does mean that treatment and prevention should be discussed early rather than waiting until symptoms become unmanageable. For women without a medical reason to avoid it, hormone replacement therapy (HRT) is often recommended after early surgical menopause and is commonly continued until at least the average age of natural menopause, around 51. Individual circumstances always take priority.
A clinician may consider blood pressure, cholesterol, weight pattern, family history, fracture risk, smoking status, activity levels and medication use alongside menopausal symptoms. Bone density scanning may be appropriate for some women, particularly where menopause occurs at a young age or there are additional osteoporosis risk factors.
HRT after surgical menopause
For many women, HRT is the most effective treatment for the symptoms and health implications of surgical menopause. It replaces some of the hormones that the ovaries previously produced, most commonly oestrogen.
If you have had your womb removed and there is no reason to use a progestogen, oestrogen-only HRT is usually prescribed. Oestrogen can be given through a skin patch, gel, spray or tablet. Transdermal options, such as patches and gel, are often favoured where there are risk factors that make tablet-based treatment less suitable, although the right choice depends on your health, preferences and response.
If you still have a womb, oestrogen needs to be paired with progestogen to protect the lining of the womb. This may be provided as oral micronised progesterone, a hormonal coil or another suitable progestogen. Where surgery was performed for endometriosis, HRT decisions can be more nuanced. Even after hysterectomy, a combined approach may be advised in some circumstances because residual endometriosis can respond to oestrogen. Specialist assessment is valuable here.
Testosterone may also be considered for persistent low sexual desire when other factors have been addressed and oestrogen treatment is optimised. It is not a general energy treatment, and it should be prescribed and monitored carefully, but it can be helpful for selected women.
HRT is not a one-size-fits-all prescription. The initial dose may need adjusting, and the best formulation can change over time. A review should consider symptom relief, side effects, bleeding where relevant, mood, sleep, sexual wellbeing and your wider health. It is reasonable to ask why a particular option has been recommended and what alternatives are available.
When HRT needs specialist input
There are situations where standard advice requires careful tailoring. This includes a personal history of breast, ovarian or endometrial cancer, active liver disease, a previous blood clot, unexplained vaginal bleeding, significant migraine changes, severe endometriosis or complex cardiovascular risk.
A diagnosis of cancer does not automatically mean that every form of menopausal treatment is off limits, but decisions should be made with the relevant oncology and specialist teams. Non-hormonal treatments can reduce flushes and night sweats for some women, while local vaginal oestrogen may be considered differently from systemic HRT. The details matter, and broad statements found online are rarely enough.
Women who have had risk-reducing ovary removal because of a genetic variant, such as BRCA, may also need a dedicated conversation about the timing and type of HRT. Their cancer-risk history, whether the womb remains, and their age at surgery all influence the plan.
Recovery: giving your body time
Recovery is not only about the surgical wounds healing. Abdominal surgery can affect mobility, bowel habits, appetite, sleep and confidence for several weeks. Follow the advice from your surgical team about driving, lifting, work and exercise. Pushing through fatigue too quickly can make recovery harder, even if you are used to caring for others or managing a demanding role.
Gentle movement, adequate protein, regular meals and hydration can support healing. Once you are cleared to do so, weight-bearing and resistance exercise are particularly valuable for bone and muscle health. This does not need to mean intensive gym sessions. Brisk walking, stair climbing, strength training and Pilates-style resistance work can all have a place, depending on your recovery and fitness.
Vaginal dryness, discomfort during sex or changes in arousal are treatable, yet often go unmentioned. Regular vaginal moisturisers and lubricants can help, while local vaginal oestrogen is highly effective for many women and has minimal systemic absorption. Pelvic health physiotherapy may be useful where pain, pelvic floor tension or bladder symptoms are part of the picture.
A practical guide to surgical menopause follow-up
The most useful follow-up is planned rather than reactive. Ideally, the discussion about menopause management begins before surgery, particularly if both ovaries are being removed before natural menopause. This gives you time to understand expected symptoms, consider HRT options and arrange a review after the operation.
After surgery, seek medical advice promptly if you have severe mood deterioration, persistent insomnia, uncontrolled flushes, new chest pain, one-sided leg swelling, heavy bleeding, fever or worsening abdominal pain. Some symptoms may relate to recovery rather than menopause, and they should not be assumed to be hormonal.
At routine reviews, bring a clear picture of what has changed. Note your sleep, mood, flushes, vaginal or urinary symptoms, libido, headaches, energy and joint pain. This helps your clinician assess whether the dose, delivery method or wider treatment plan needs to change. Blood tests are not routinely needed to diagnose surgical menopause after both ovaries have been removed, but they may be useful in selected situations or to investigate other causes of ongoing symptoms.
The goal is not simply to tolerate an abrupt change in hormones. With informed treatment choices, monitoring and support for your physical and emotional wellbeing, many women regain comfort, energy and confidence after surgical menopause.
